Gleason score and Grade Group
The prostate cancer grading system: the pathologist scores the two most common growth patterns from 3 to 5 and adds them (6 to 10), now simplified into Grade Groups 1 to 5. Grade Group 1 (Gleason 6) is so indolent that many argue it should not be called cancer.
Gleason 3+3=6 (Grade Group 1) is managed by active surveillance; 3+4 (Group 2) is favourable intermediate; 4+3 (Group 3) unfavourable; 8 (Group 4) and 9-10 (Group 5) are high risk and receive radiotherapy with long ADT or surgery with adjuvant therapy. Grade, PSA and stage combine into NCCN/D'Amico risk groups that drive nearly every treatment decision; genomic classifiers (Decipher) and AI pathology (ArteraAI) now refine them. Inter-observer variability is real, and MRI-targeted biopsy has reduced under-grading.
