MGUS and smouldering myeloma

The two stages before multiple myeloma: MGUS is an abnormal antibody in the blood with no symptoms (common and mostly harmless), smouldering myeloma has more abnormal cells but still no organ damage. Both are watched; high-risk smouldering disease is now sometimes treated early.

MGUS is present in about 3% of people over 50 and progresses to myeloma at ~1% per year; smouldering myeloma (≥10% marrow plasma cells or M-protein ≥30 g/L, without CRAB features) progresses at ~10% per year for five years, faster with the 20/2/20 high-risk criteria. The M-protein (paraprotein) and serum free light chains are the measured markers. Lenalidomide (E3A06) and daratumumab (AQUILA, 2024) delay progression in high-risk smouldering disease, prompting debate about treating a pre-cancer; screening for MGUS (iStopMM) is being evaluated in Iceland. Nearly all myeloma passes through MGUS.
