Gleason patterns 3-5 summed (6-10) and mapped to ISUP Grade Groups 1-5. Grade Group 1 (Gleason 6) is now managed by surveillance; cribriform and intraductal patterns carry extra risk. AI graders (Paige, ArteraAI) reduce inter-observer variation.
