Histologic transformation
When a lung adenocarcinoma escapes targeted therapy by turning into a different cell type, usually small-cell.
Small-cell (and squamous) transformation occurs in ~5-15% of EGFR-TKI resistance, especially with baseline RB1 and TP53 loss; also after ALK and KRAS inhibitors. Requires re-biopsy (not detectable by ctDNA genotyping) and a switch to platinum-etoposide.
