EGFR C797S

A mutation that stops osimertinib from binding to EGFR; the main on-target way lung cancers escape it.

Arises in ~10-15% of osimertinib progressors, in cis or trans with T790M. No approved fourth-generation TKI yet (BLU-945 and others discontinued or early); amivantamab-based and ADC-based regimens are the practical answer. Detectable by ctDNA.
