Treat cachexia before it starts
Wasting kills many cancer patients and makes treatment impossible. With the first effective anti-cachexia drug in phase 3, the next question is whether starting it early prevents wasting rather than reversing it.
Ponsegromab reversed weight loss in established cachexia (2024). GDF-15 rises early in pancreatic and lung cancer; preventing muscle loss could improve chemotherapy tolerance and survival.
Hypothesis
Anti-GDF-15 therapy started at diagnosis in high-GDF-15 patients preserves muscle mass and increases chemotherapy completion and overall survival.
Rationale
Muscle loss is largely irreversible late; GDF-15 is measurable and causal; ponsegromab is safe.
What would test it
Randomised phase 3 of ponsegromab vs placebo from first-line chemotherapy in metastatic pancreatic cancer with elevated GDF-15, endpoints OS and relative dose intensity.
