A PET scan after two cycles of antibodies alone identifies women who can be cured without chemotherapy.
PHERGain: PET responders continued trastuzumab + pertuzumab (± endocrine therapy) alone; those with pCR skipped chemotherapy, with 3-year iDFS 95.4%. Phase 3 PHERGain-2 and similar response-adapted designs (WSG-ADAPT) are testing the approach more broadly.
